BEFORE THE NOTARY PUBLIC AFFIDAVIT

We, ______________________________________________ (the “Applicant”) and the Applicant’s Parent(s)/Legal Guardian(s), ______________________________________________, residing at ______________________________________________, being the holder(s) of Indian Passport/Aadhaar No. ______________________________________________, do hereby solemnly affirm, declare and state as follows:

1. ADMISSION AND STUDIES

The Applicant has applied, through Fly Future Education Pvt. Ltd. (FFE), for admission to and study in the six-year Specialist Course in General Medicine (31.05.01) at the International Faculty of General Practitioners of the Federal State Budgetary Educational Institution of Higher Education ------------------------------, Arkhangelsk, Russian Federation, under the Ministry of Health of the Russian Federation, after successfully completing the applicable entrance examination requirements.

The said course is conducted in the English medium for foreign nationals and is intended to provide medical education leading to the qualification applicable to the relevant regulations in India.

2. PAID SERVICES OF FLY FUTURE EDUCATION PVT. LTD.

We hereby agree to avail ourselves of the admission-related paid services provided to Fly Future Education Pvt. Ltd. (FFE).

The total service charges payable for the said services shall be:

?1,20,000/- + GST and OTC USD 1,500

The above payment includes the following services:

  • Assistance with the Applicant’s admission process.
  • Apostille of the Applicant’s Senior Secondary School documents.
  • Notarized translation of the Passport, Senior Secondary School documents and NEET result.
  • Assistance with the process of obtaining the Entry Study Visa for the Russian Federation.

3. PAYMENT TERMS

The detailed payment terms, instalment amounts and applicable payment due dates shall be governed by the relevant Fee Structure and/or Agreement executed between the parties.

 

4. NON-REFUNDABLE FFE SERVICE CHARGES

The total service charges payabl to Fly Future Education Pvt. Ltd. (FFE) shall be Non-Refundable, Non-Adjustable, Non-Claimable and Non-Recoverable, subject to the applicable Agreement and mutually agreed terms and conditions.

No refund, adjustment or recovery claim shall be made in respect of the service charges paid to FFE where the applicable Agreement and terms specifically provide that such charges are non-refundable.

This shall include, but shall not be limited to, the following circumstances:

  • Rejection of the visa by the Russian authorities.
  • Rejection or denial of admission by the University.
  • Withdrawal by the Applicant from the admission or study process at any stage.
  • Force Majeure circumstances.
  • Changes in University policies.
  • Changes in Immigration Laws or regulations.
  • Personal circumstances of the Applicant.
  • Any other reason or circumstance covered under the applicable Agreement and terms.

We agree that, subject to the applicable Agreement and terms, we shall not raise any claim for refund, adjustment or recovery of the FFE service charges.

5. UNIVERSITY TUITION AND HOSTEL FEES

The annual tuition fee and hostel/accommodation fee shall be determined in accordance with the applicable orders issued by the Rector of the University.

As per Rector’s Order  dated 10.04.2025, the tuition fee for first-year International Students enrolled in the General Medicine Full-Time English Medium programme for the academic year 2026–2027 was stated to be RUB 350,000.

The accommodation/hostel fee applicable to International Students shall be determined in accordance with the applicable Rector’s Order, including Rector’s Order dated 25.10.2026, as applicable.

The tuition fee and accommodation/hostel fee may be increased in accordance with applicable University regulations and the inflation rate considered under the Federal Budget of the Russian Federation for the relevant financial year and planning period.

6. PAYMENT OF UNIVERSITY FEES

We understand and agree that all tuition fees and accommodation/hostel fees payable to the University shall be paid directly by us to ------------------.

Fly Future Education Pvt. Ltd. (FFE) shall not collect, retain or be responsible for University fees in any manner, unless expressly provided otherwise under a separate written agreement.

We undertake to pay the applicable fees for the relevant academic year in accordance with the University's prescribed payment schedule and within the stipulated due date.

We confirm that we have been informed about the tuition fees, hostel/accommodation charges and applicable payment deadlines of the University. We have also reviewed the relevant fee-related information available on the Fly Future Education Website www.flyfuture.in.

7. AUTHENTICITY OF DOCUMENTS

It shall be our sole responsibility to provide all required documents and certificates in genuine, authentic and valid form, together with all necessary seals, attestations and verifications, wherever applicable.

We shall be solely responsible for all consequences arising from the submission of any false, incorrect, forged or unauthenticated document or information.

8. NMC REGULATIONS

We acknowledge and understand the applicable regulations of the National Medical Commission (NMC) governing Indian citizens pursuing medical education outside India, including the Foreign Medical Graduate Licentiate (FMGL) Regulations, 2021, as applicable.

We declare that Fly Future Education Pvt. Ltd. (FFE) has provided us with the available relevant information concerning the aforesaid institution.

We have reviewed and understood the relevant information regarding the University, course, faculty, tuition fees, accommodation and other related matters before executing this Affidavit/Declaration.

9. RESPONSIBILITY FOR PROVIDING DOCUMENTS AND REQUIRED MATERIALS

We agree that the Applicant shall provide all documents, information and other materials required for admission, visa processing, University procedures and other applicable formalities within the prescribed time.

The Applicant and his/her Parent(s)/Legal Guardian(s) shall be responsible for the accuracy, authenticity, completeness and timely submission of all documents and information provided by the Applicant.

10. COMPLIANCE WITH THE LAWS OF THE RUSSIAN FEDERATION AND UNIVERSITY RULES

We agree that the Applicant shall:

  • Comply with the Constitution and laws of the Russian Federation.
  • Comply with all applicable entry and exit requirements of the Russian Federation.
  • Comply with all applicable rules governing residence and travel within the Russian Federation.
  • Comply with all internal rules and regulations of the University and hostel/accommodation.
  • Complete the selected course in accordance with the prescribed academic rules and requirements.
  • Attend classes and lectures regularly.
  • Appear for all examinations, credits and assessments within the prescribed schedule.
  • Participate in practical training as required by the course.
  • Use University property, books, equipment and other facilities responsibly and carefully.
  • Comply with all lawful instructions issued by the University and relevant governmental and immigration authorities.

11. PROHIBITED ACTIVITIES

We confirm that the Applicant shall:

  • Not consume alcohol or prohibited substances in violation of applicable laws or University/hostel rules.
  • Not use, possess or engage in any activity involving narcotic or other prohibited substances.
  • Not participate in ragging or any similar prohibited activity.
  • Not engage in any misconduct, misbehaviour or unlawful activity within the hostel, hospital, University campus or any other place.
  • Not violate the laws of the Russian Federation or the rules and regulations of the University.

The Applicant shall be responsible for the consequences arising from any violation of applicable laws, rules or regulations.

12. EXPULSION FROM THE UNIVERSITY

If the Applicant is expelled or dismissed from the University, the Applicant shall be required to leave the Russian Federation within the applicable period and in accordance with the laws, immigration requirements and University regulations then in force.

13. ACADEMIC PERFORMANCE AND VIOLATION OF RULES

If the Applicant’s academic performance is found to be unsatisfactory, attendance is inadequate, or the Applicant violates the rules of the University, hostel, Immigration Authorities or the laws and regulations of the Russian Federation, the University may take appropriate action in accordance with applicable laws and University regulations.

Such action may include suspension, cancellation of admission, dismissal or expulsion, as applicable.

In such circumstances, any refund of fees shall be governed by the applicable laws, University rules and the relevant Agreement.

14. MEDICAL EXAMINATION AND INSURANCE

Upon arrival in the Russian Federation, the Applicant shall undergo all medical examinations and procedures required under the applicable laws of the Russian Federation and the rules of the University.

The Applicant shall maintain valid medical insurance throughout the period of study, as required by applicable laws and University regulations.

It shall be the Applicant’s responsibility to complete all required medical tests, vaccinations and other health-related formalities within the prescribed time.

15. FINGERPRINT AND PHOTOGRAPH REGISTRATION

Upon the Applicant’s first entry into the Russian Federation, the Applicant shall complete the required fingerprint and photograph registration in accordance with the applicable laws of the Russian Federation.

The Applicant shall comply with all instructions and requirements issued by the relevant Migration Authorities.

16. STUDY VISA EXTENSION

During the period of study, the Applicant shall ensure timely extension/renewal of the Study Visa in accordance with the applicable laws and regulations of the Russian Federation.

The Applicant shall be responsible for submitting all required documents and paying all applicable fees within the prescribed time limits for visa extension/renewal.

17. PERSONAL MEDICAL BOOK

During the period of study, the Applicant shall maintain the required Personal Medical Book and/or medical records in accordance with the applicable requirements of the University and the laws of the Russian Federation.

18. LIABILITY AND INDEMNIFICATION

We acknowledge that we shall be responsible for any liability arising from the submission of false, incorrect, forged or unauthenticated documents, as well as from any misconduct, violation of rules, unlawful act, omission or other conduct of the Applicant.

We further acknowledge that matters relating to Visa Rejection, Immigration Issues, University Admission Denial, Expulsion or Force Majeure shall be subject to the decisions of the relevant governmental authorities, the University and the applicable laws and regulations.

To the extent permitted by applicable law and the relevant Agreement, we shall be responsible for and shall indemnify the concerned parties against losses, liabilities, claims or consequences arising from the Applicant’s acts, omissions, misrepresentation or violation of applicable laws and regulations.

19. DISPUTES AND JURISDICTION

Any dispute arising out of or in connection with this Affidavit, the services provided by Fly Future Education Pvt. Ltd., or any related matter shall be governed by the applicable law and subject to the jurisdiction specified in the relevant Agreement, to the extent legally permissible.

20. FINAL DECLARATION

The contents of this Affidavit have been read over and explained to us in a language understood by us, and we have fully understood the contents thereof.

We hereby declare that all information and statements provided by us are true and correct to the best of our knowledge and belief.

We further confirm that we have read, understood and agreed to the terms, conditions, responsibilities and declarations contained in this Affidavit.
